Rebuilding Aster’s Operational Backbone, One System at a Time

Hospitals like to talk about compassion. Sudhindra G Bhat prefers to talk about systems because, in his experience, compassion that isn’t backed by a system is just a good intention waiting to fail someone.

As COO of Aster CMI Hospital, Bengaluru, Sudhindra occupies the unglamorous but unforgiving seat of healthcare leadership: the one where clinical ambition meets balance sheets, where a doctor’s vision for a new department must survive contact with staffing rosters and capital budgets. Over a career spanning multiple hospitals and now Aster CMI, Sudhindra has built a reputation less for grand pronouncements and more for the kind of operational discipline that hospitals rarely get credit for, until something goes wrong and it turns out, at his institutions, it doesn’t.

The Green Heart Programme made Aster the first hospital in the country to formally institutionalise caregiver wellbeing as a clinical safety measure. His guiding principle, that patient experience can never exceed caregiver experience, has quietly reordered how leadership teams think about burnout, retention, and the invisible economics of empathy. 

In this conversation, Sudhindra talks about the tension between clinical vision and financial reality, why he loses sleep over communication breakdowns rather than budgets, and why he’s actively building an institution designed to outlast him.

As COO, how do you translate a doctor’s vision of ideal care into an operationally and financially sustainable model? Where does that tension get most uncomfortable?

At Aster CMI, the patient comes first, always. Every clinical vision begins with one question: how will this improve outcomes and experience? My job is to work with our clinicians to turn that vision into a model that can deliver high-quality care consistently, not just once.

That means balancing clinical excellence, accessibility, operational efficiency, and financial responsibility, usually all at once. We bring in multidisciplinary teams to assess infrastructure, workforce planning, technology, and long-term value before any major decision is made.

The tension gets sharpest when a clinical vision demands upfront investment in advanced technology or specialised infrastructure. In those moments, we lean on our core value of excellence while staying disciplined about resources. The goal is never to compromise care; it’s to build systems that enable us to deliver exceptional care sustainably for years, not just for one budget cycle.

Can you share a strategic initiative you led that significantly advanced your organisation’s clinical excellence and innovation?

One of the most significant strategic initiatives I have had the privilege of leading was the launch of the ROSA ONE Brain and Mazor X Spine robotic platforms at Aster CMI Hospital, a major milestone in advancing our neurosciences program. Driven by a vision to bring world-class robotic technology to our patients, the initiative empowered our clinical teams to deliver safer, more precise, and minimally invasive brain and spine surgeries.

In 2026, Aster CMI Hospital became the first hospital in Karnataka to launch an integrated robotic Brain & Spine Surgery suite, and introduced India’s first doctor-led integrated robotic platform for brain and spine surgery, combining ROSA ONE Brain for cranial neurosurgery, Mazor X for robotic spine surgery, and O-arm intraoperative imaging.

This initiative reinforced our commitment to clinical excellence, innovation, and patient-centric care, setting new benchmarks in precision, safety, and outcomes for complex neurological and spinal procedures.

What operational failure point keeps you up at night, and how have you built systems around it?

Patient safety is non-negotiable, and the area that demands constant vigilance is communication across the continuum of care. Multiple teams work around the clock, and any breakdown during handovers, critical transitions, or emergencies can affect outcomes.

We’ve built standardised communication protocols, multidisciplinary rounds, incident reporting, quality audits, digital health tools, and continuous training around exactly this risk. Just as important is a culture where every team member feels safe raising concerns. Operational excellence isn’t about perfection; it’s about continuous learning.

What has permanently changed about how the hospital operates after COVID, and what did you choose not to bring back?

The pandemic taught us agility, resilience, and collaboration and accelerated our shift toward telemedicine, virtual consultations, and data-driven decision-making. It also put employee wellbeing, workforce flexibility, and emergency preparedness permanently on our operational agenda.

What we didn’t bring back was rigid, hierarchical decision-making. COVID showed us the value of empowering teams and moving fast without losing sight of patient safety.

How do you build an operationally resilient institution that doesn’t depend on a single leader?

Beyond a title, leadership is a responsibility distributed across every level. We invest in strong governance, succession planning, and leadership development, so knowledge-sharing and accountability become part of the culture, not exceptions to it.

Our goal is leaders who create other leaders. A truly resilient healthcare organisation is one where care remains seamless regardless of who’s in the corner office, because values, culture, and systems are stronger than any one individual, including me.
